Endoscopic vs. Classic Facelift:
A Detailed Comparison
This isn't a question of "which is better" — it's a question of "which is right for you." Each technique has its ideal candidates, advantages and limitations. Here is an honest comparison.
Core Principles: Same Philosophy, Different Path
Both endoscopic and classic Deep Plane facelift share the same fundamental philosophy: not tightening skin, but repositioning deeper structures. The difference lies in how those structures are accessed and which structures can be addressed.
Endoscopic approach: camera and instruments through small openings hidden in the hairline, no ability to remove excess skin — but a minimal surgical footprint.
Classic Deep Plane approach: direct visualisation through standard incisions, full access to all structures, more visible but well-hidden incisions.
Comprehensive Comparison
| Criterion | Endoscopic | Classic Deep Plane |
|---|---|---|
| Incision length | 1–2 cm, in hairline | 8–12 cm, hidden in hair and in front of / behind ear |
| Scar visibility | Practically invisible | More visible in first 3–6 months, then fades |
| Procedure duration | 2–3 hours | 4–6 hours |
| Return to public | 7–10 days | 14–21 days |
| Full recovery | 3–4 weeks | 6 weeks |
| Upper third (forehead, brows) | Excellent access | Can be combined with endo. browlift |
| Mid-face (cheeks) | Good access | Excellent access |
| Lower third (jowls, jawline) | Limited | Excellent |
| Neck | Not addressed | Excellent |
| Skin excess | No excision | Excision where needed |
| Longevity of results | 5–10 years | 10–15 years |
| Ideal age | 30s and early 40s | 40–70 |
Who Is the Ideal Candidate for Endoscopic?
Endoscopic mid-facelift / browlift is optimal when:
- The patient has moderate changes in the upper and mid-face
- Deeper nasolabial folds are present
- The lower third and neck are not yet significantly affected
- Skin elasticity is good to excellent — skin can adapt on its own after tissue lifting, without excision
- The patient prefers a shorter recovery and minimal surgical trace
- Age: 25–45 for a preventive approach
Who Is the Ideal Candidate for Classic Deep Plane?
Deep Plane facelift is optimal when:
- There is pronounced jowling and loss of jawline definition
- The neck shows platysmal bands, submental fullness or skin excess
- Descent is present across all three thirds of the face
- Skin elasticity is reduced — skin cannot adapt without excision
- The patient wants the most durable possible result
- Age: 40–75 — though there is no strict upper limit
There is no "superior" technique. There is the right technique for your face. Our job is to identify which approach delivers the optimal result for your specific anatomical situation.
The Hybrid Approach: Combining Both Techniques
In practice, a frequent combination is: endoscopic browlift for the upper third + classic Deep Plane for the mid-face, lower third and neck. The patient achieves the smallest possible overall surgical trace alongside a comprehensive result.
This is particularly popular with patients who have marked changes in the forehead and brows (descended brows, horizontal forehead lines) alongside changes in the lower third and neck.
Pricing and Costs
The endoscopic procedure is usually somewhat less expensive than a full Deep Plane facelift — it takes less time and requires less anaesthesia. A hybrid combination falls in between.
At Poliklinika Malić, everything is included in one price: consultation, procedure, anaesthesia, hospitalisation and post-operative follow-ups. No hidden costs.
How We Determine What Is Right for You
At your consultation we work through a structured facial analysis:
- Photographic documentation in standardised views
- Physical examination — assessment of skin elasticity, degree of tissue laxity, fat compartments
- Discussion of goals and realistic expectations
- A procedure recommendation with an explanation of why it suits your anatomy
No rush, no pressure. The decision is yours — our job is to give you every piece of information you need to make it with confidence.
